PHOENIX — "Give me your hands. Don't let him get inside." Phoenix Police says a 40mm round was ineffective, a second officer tackled the armed man and the man's gun discharged during the fall, striking both the man and the officer. The man died; the officer survived. Voss finding: audit each force transition separately—launcher effect, containment, movement toward the home, tackle mechanics, muzzle position, backdrop and foreseeable crossfire. The administrative investigation is ongoing; no legal or policy conclusion has been made. Source captions checked against the published audio.
